When should I refer a patient concerned about their memory or cognition?

As dementia is one of the most feared diagnoses and with healthy brain function such a fundamental part of a good life, you may find your patients bringing this issue up more and more, even from a younger age than expected. It is reasonable to refer anyone with these concerns for a number of diverse reasons.

Dementia diagnoses are often delayed by years, delaying management that can make a difference to the patient and their family in numerous ways. Anxiety about cognitive changes can be very significant for some and our assessments are often helpful in addressing this. Lastly, risk reduction and intervention is more effective when commenced early.

When is it appropriate to refer for geriatric evaluation and management?

Caring for older patients who may be experiencing cognitive change, functional impairment, driving concerns, falls, frailty or polypharmacy can take a lot of your time. This is a part of her work which Dr Ireland very much enjoys; she is able to offer long consults with these patients, and their relatives, to assist in streamlining the care you provide and helping them to address their health priorities.

Managing the earliest signs of frailty, balance changes/falls and decline in cognition is where we can make the biggest impact, so we are happy to see any of your older patients who might benefit from a little more time.

Please note, inhouse clinics provides an outpatient service only and does not offer acute or rehab admissions. Dr Ireland can provide visits to private homes and residential care facilities where distance permits.
